Phyllis Joan Koslosky of Chatsworth passed away on September 30, 2023, she was born on January 20, 1951.
She is preceded in death by her parents, Joe, and Alma Ledford.
She is survived by her daughter and son-in-law, Tina, and Jeff Fields of Chatsworth; grandchildren, Curtis and Tyler Fields of Chatsworth, Derek (Tiffany) Fields of Calhoun; great-grandchildren, Chase, and Riley Fields.
She will be lovingly missed and remembered. May she rest in peace.
